Millcreek of Pontotoc accepts admission inquiry calls directly from parents/guardians, local mental health facilities, acute care facilities and school districts. We carefully screen each prospective admission and strive to admit those who will benefit from our program. | Criteria for Residential Treatment |
- Patient must be between the ages of 6-21 years.
- Patient must have had psychiatric treatment within the last 60 days.
- Patient must have had an evaluation completed within the last 60 days that recommends residential treatment (must be completed by either a child and adolescent psychiatrist or a licensed clinical psychologist).
- Patient must have IQ scores with a Full Scale IQ of 60 or above.

| Criteria for Admission to Day Treatment |
- Patient must be enrolled in school or participating in a GED program.
- Patient must have a current Special Education Eligibility.
- Patient must be between the ages of 6-21 years (must have turned 21 by September of the current school year).
- The IEP committee of the referring school district should have met to determine that Millcreek Day Treatment is the patient's least restrictive environment, then make a referral.
- The school district should send copies of the students IEP and Eligibility and Summary/Assessment Team Report to the Admissions Team at Millcreek of Pontotoc for review.
- The school district should fill out an Educable Child Application to be sent to the Department of Education.
